Insulin-like growth factor 1 is produced largely in the liver in response to growth hormone signaling. Its concentration shifts over days rather than minutes, which makes it practical for tracking changes across a study period. Interpretation still depends on age, nutritional status, and concurrent illness, all of which independently affect the marker. Reference ranges are therefore stratified, and comparisons are usually made within an individual over time rather than against a single population threshold.
Assays for these markers differ in calibration and antibody specificity, so results from different platforms are not always interchangeable. Reported values can shift when a laboratory changes method, even without any biological change. Studies that span long periods or multiple sites often need cross-validation of assays. This methodological variability is a recognized limitation when comparing findings across published reports, and it remains a topic of ongoing standardization work.
Stability testing examines how the molecule changes under controlled stress. Thermal stress, light exposure, and extremes of pH are applied separately so that each degradation route can be attributed to a specific cause. The main observed changes are oxidation, deamidation, and aggregation into dimers or higher-order species. Accelerated studies at elevated temperature are used to estimate behavior over longer periods, though such extrapolation carries uncertainty. For a lyophilized powder, residual moisture and the choice of bulking agent strongly influence how quickly these changes appear.
Practical handling centers on limiting moisture, oxygen, and temperature excursions. Lyophilized material is generally held at or below minus twenty degrees Celsius, protected from light and kept sealed until use. Once reconstituted, solutions are typically kept cold and used within a short window because hydrolysis and microbial growth both accelerate in liquid form. Repeated freeze-thaw cycles are avoided, since they promote aggregation. Vial contents should be inspected for particulates and clarity before analysis, and working aliquots are prepared to reduce the number of times the stock is opened.

Lyophilized tesamorelin is generally stored refrigerated at temperatures between 2 and 8 degrees Celsius. The solid form is comparatively stable when kept dry and protected from light. Moisture uptake can promote aggregation and degradation, so sealed containers with desiccant are common. Researchers typically avoid repeated temperature cycling, which may stress the peptide. Documentation accompanying reference materials usually specifies a shelf life under these conditions.
Once reconstituted, the peptide is handled as a solution and is less stable than the lyophilized powder. Aqueous solutions are commonly kept cold and used within a defined period. Buffer composition and pH influence degradation rates, with extremes of acidity or alkalinity accelerating hydrolysis. Preservatives may be added in multi-dose formats to limit microbial growth. Freezing and thawing of solutions is generally avoided because it can cause precipitation or loss of activity.
Tesamorelin binds to growth hormone-releasing hormone receptors on somatotroph cells in the anterior pituitary. Receptor activation increases intracellular cyclic AMP and promotes synthesis and secretion of growth hormone. Because the peptide mimics endogenous GHRH, it amplifies the normal pulsatile release of growth hormone rather than providing exogenous growth hormone directly. This upstream action distinguishes tesamorelin from recombinant growth hormone preparations and from growth hormone secretagogues that act at different receptors.
Stimulated growth hormone release leads to hepatic production of insulin-like growth factor 1, a key mediator of many growth hormone effects. In clinical studies, tesamorelin increased IGF-1 levels in a dose-dependent manner, although the response varies among individuals. The drug's effect on visceral fat is thought to involve growth hormone-mediated lipolysis and altered adipocyte metabolism. Muscle mass and lean body mass have also been assessed as secondary outcomes, but changes are generally smaller and less consistent than fat reductions.
Pharmacodynamic studies show that tesamorelin reduces visceral adipose tissue more than subcutaneous adipose tissue in the studied population. This selectivity may relate to differences in blood flow and hormone sensitivity between fat depots. Effects on glucose metabolism and insulin sensitivity have been investigated, with some trials reporting modest changes and others showing stability. The precise relationship between growth hormone exposure, IGF-1 levels, and visceral fat loss remains an active area of analysis.
Clinical endpoints or clinical outcomes are outcome measures referring to occurrence of disease, symptom, sign or laboratory abnormality constituting a target outcome in clinical research trials. The term may also refer to any disease or sign that strongly motivates withdrawal of an individual or entity from the trial, then often termed a humane (clinical) endpoint. The primary endpoint of a clinical trial is the endpoint for which the trial is powered. Secondary endpoints are additional endpoints, preferably also pre-specified, for which the trial may not be powered. Surrogate endpoints are trial endpoints that have outcomes that substitute for a clinical endpoint, often because studying the clinical endpoint is difficult, for example using an increase in blood pressure as a surrogate for death by cardiovascular disease, where strong evidence of a causal link exists.
Unlike most tubers, but in common with many other members of the Asteraceae (including the artichoke), Jerusalem artichoke tubers store their carbohydrates as the polysaccharide inulin (not to be confused with the peptide insulin) rather than as starch. This has made them an important source of inulin used as a dietary fiber in food manufacturing. Jerusalem artichoke can propagate with seeds and tubers but the use of tubers leads to higher yields. For planting, the tubers are cut into pieces with three to five buds that are placed in 5–10 centimetres (2–4 in) depth in the soil. Jerusalem artichoke has low nutrient requirements and needs less nitrogen than other energy crops. The competitiveness against weeds is high, making weed control easier but also making it harder to grow a different culture afterward, since some small tubers usually remain in the ground after harvest. The plant's high competitiveness may be due to allelopathic effects, high plant size, and rapid growth rate. Crop yields are high, typically 16–20 tonnes per hectare (7–9 short ton/acre) for tubers, and 18–28 tonnes per hectare (8–12 short ton/acre) green weight for foliage. Tubers remaining in the ground lie dormant over winter and can handle temperatures as low as −30 °C (−22 °F). Jerusalem artichoke also has potential for production of ethanol fuel, using inulin-adapted strains of yeast for fermentation. The tubers are used for cooking and baking in the same ways as potatoes, but unlike the potato, they can also be eaten raw.
Dihydroxyphenylalanine ammonia-lyase was presumed to use 3,4-dihydroxy-L-phenylalanine (L-DOPA) to produce trans-caffeate and NH3. However, the EC number for this purported enzyme was deleted in 2007, as no evidence has emerged for its existence.
The education system of Malaysia features a non-compulsory kindergarten education followed by six years of compulsory primary education and five years of optional secondary education. Schools in the primary education system are divided into two categories: national primary schools, which teach in Malay (i.e.:Sekolah Kebangsaan Sungai Binjai, SK Bukit Tiu), and vernacular schools, which teach in Chinese or Tamil (i.e.:SJK(C) Pin Hwa 2, SJK(T) Bandar Mentakab). Secondary education (i.e.:SMK Sura, Kajang High School) is conducted for five years. In the final year of secondary education, students sit for the Malaysian Certificate of Education examination. Since the introduction of the matriculation programme in 1999, students who completed the 12-month programme in matriculation colleges can enrol in local universities. However, in the matriculation system, only 10% of places are open to non-bumiputera students.

Therefore almost all patients will in addition require immunosuppressants, such as prednisone, mycophenolate or azathioprine, as acetylcholinesterase inhibitors alone are insufficient. The surgical removal of the thymus may improve symptoms in certain cases. Plasmapheresis and high-dose intravenous immunoglobulin may be used when oral medications are insufficient to treat severe symptoms, including during sudden flares of the condition. On rare occasions the respiratory muscles can become significantly weak, subsequent mechanical ventilation may then be required. Once intubated, acetylcholinesterase inhibitors may be temporarily withheld to reduce airway secretions. Myasthenia gravis affects 50 to 200 people per million. It is newly diagnosed in 3 to 30 people per million each year. Increased awareness has made diagnosis more common. Myasthenia gravis most commonly occurs in women under the age of 40 and in men over the age of 60. It is uncommon in children. With treatment, most have fulfilling lives and have a normal life expectancy. The word is from the Greek mys, "muscle" and asthenia "weakness", and the Latin gravis, "serious".
The Court of Justice held that the Directive could be relied on by her because equality was also a general principle of EU law. Fourth, if the defendant is an emanation of the state, even if not central government, it can still be bound by Directives. In Foster v British Gas plc the Court of Justice held that Mrs Foster was entitled to bring a sex discrimination claim against her employer, British Gas plc, which made women retire at age 60 and men at 65, if (1) pursuant to a state measure, (2) it provided a public service, and (3) had special powers. This could also be true if the enterprise is privatised, as it was held with a water company that was responsible for basic water provision. Fifth, national courts have a duty to interpret domestic law "as far as possible in the light of the wording and purpose of the directive". Textbooks (though not the Court itself) often called this "indirect effect". In Marleasing SA v La Comercial SA the Court of Justice held that a Spanish Court had to interpret its general Civil Code provisions, on contracts lacking cause or defrauding creditors, to conform with the First Company Law Directive article 11, that required incorporations would only be nullified for a fixed list of reasons. The Court of Justice quickly acknowledged that the duty of interpretation cannot contradict plain words in a national statute. But, if a member state has failed to implement a Directive, a citizen may not be able to bring claims against other non-state parties. It must instead sue the member state itself for failure to implement the law.
==== Gás do Povo ==== In November 2025, the Gás do Povo program began, replacing Auxílio Gás. The program guarantees the free refill of the LPG cylinder (13 kg) at retailers accredited by the government. The measure aims to promote access to LPG (Liquefied Petroleum Gas), avoiding the use of precarious alternatives such as firewood, charcoal, and kerosene, which can expose families to toxic smoke, chronic respiratory diseases, and the constant danger of burns. The benefit is intended for families registered in the Cadastro Único (CadÚnico), with an income of up to half the minimum wage, with priority given to those who receive Bolsa Família. Caixa Econômica Federal is responsible for the operationalization of the benefit, being in charge of distributing the refill vouchers, registering participating retailers, and validating the beneficiaries' means of access.

Thorium (90Th) has seven naturally occurring isotopes but none are stable. One isotope, 232Th, is relatively stable, with a half-life of 1.40×1010 years, considerably longer than the age of the Earth, and even slightly longer than the generally accepted age of the universe. This isotope makes up nearly all natural thorium, so thorium was considered to be mononuclidic. However, in 2013, IUPAC reclassified thorium as binuclidic, due to large amounts of 230Th in deep seawater. Thorium has a characteristic terrestrial isotopic composition and thus a standard atomic weight can be given. Thirty-one radioisotopes have been characterized, with the most stable being 232Th, 230Th with a half-life of 75,400 years, 229Th with a half-life of 7,916 years, and 228Th with a half-life of 1.91 years. All of the remaining radioactive isotopes have half-lives that are less than thirty days and the majority of these have half-lives that are less than ten minutes. One isotope, 229Th, has a nuclear isomer (or metastable state) with a remarkably low excitation energy, recently measured to be 8.355733554021(8) eV It has been proposed to perform laser spectroscopy of the 229Th nucleus and use the low-energy transition for the development of a nuclear clock of extremely high accuracy. The known isotopes of thorium range in mass number from 207 to 238.
== Safety and side effects == Some anti-obesity medications can have severe, even lethal side effects, fen-phen being a famous example. Fen-phen was reported through the FDA to cause abnormal echocardiograms, heart valve problems, and rare valvular diseases. Out of 25 anti-obesity medications withdrawn from the market between 1964 and 2009, 23 acted by altering the functions of chemical neurotransmitters in the brain. The most common side effects of these drugs that led to withdrawals were mental disturbances, cardiac side effects, and drug abuse or drug dependence. GLP-1 receptor agonists are known to have common side effects including nausea and vomiting. Deaths were associated with seven products. Ephedra was removed from the US market in 2004 over concerns that it raises blood pressure and could lead to strokes and death.
== Phone intercepts == In September 2008, Georgia presented to the public recordings of intercepted phone calls made on 7 August by Ossetian border guards on a Georgian cellular network, which were then independently translated from the original Ossetian language by The New York Times. The recordings, allegedly proving that entry of Georgian troops into Tskhinvali was preceded by movement of part of a Russian armoured regiment into South Ossetia nearly a day earlier, were reviewed and assessed as valid by senior American government and military officials. According to a call recorded at 3:52 am on 7 August, a South Ossetian servicemen at the tunnel with the surname Gassiev was asked by a supervisor at the headquarters if the armor had arrived, to which he replied: "The armor and people." The guard also said that the people had gone through, while "they had already arrived" 20 minutes earlier. Gassiev told the supervisor in the first call at 3:41 am that an inspection of military vehicles inside the tunnel had been requested by the Russian colonel, but Gassiev did not know the exact identity of the colonel. Gassiev reported the exit of armored vehicles from the tunnel to the supervisor at 3:52 am. Authenticity of the calls was not questioned by Russia. Colonel Andrei Kazachenko who was mentioned in the recording, belonged to the 135th Motorized Rifle Regiment, according to Russian media reports after the war.

It varies slowly and reflects cumulative axis activity rather than momentary secretion. Growth hormone is released in pulses affected by sleep, stress, and meals, making single readings hard to interpret. The slower marker gives a more stable picture across a study period.
Assay calibration and antibody specificity differ between platforms, so identical samples can yield different numbers. A method change within one laboratory can shift results without any biological change. Cross-validation is often needed for multi-site work.
They capture only one moment in a pulsatile pattern and are strongly influenced by recent activity and meals. Repeated sampling or overnight profiles provide a more representative view. Provocative testing is an alternative when a dynamic response is of interest.
Reversed-phase high-performance liquid chromatography with ultraviolet detection is the standard technique for purity and content. Mass spectrometry provides orthogonal confirmation of identity. The two are normally used together rather than in isolation.
